NDIS Support Coordination
A Support Coordinator works alongside you and/or your family to:
- Build your ability to understand and implement your NDIS plan
- Maintain relationships with your service providers
- Manage your service delivery
- Provide support for you to live your best life
This is achieved by:
- Helping you understand how to use your NDIS plan
- Teaching you how to use the NDIS portal to monitor your funds
- Managing your NDIS services to get the best outcome from your plan
- Negotiating with services, both formal and informal, around their supports for you and the cost
- Monitoring the progress and outcomes of your goals
- Helping you prepare for your next NDIS review
- Working with your support network (family, friends and other service providers) to help you meet your goals set out in your NDIS plan
To access our service, your Support Coordination funding needs to be plan managed or self-managed.
